2. Fill out this Online form and fax it to us
Fill in all the fields in as much detail as possible. Then click "Submit" and you will receive a reference number . You must keep this reference number, and write it on a piece of paper. When you mail us your drive, be sure to include this number. This will help us match your form with your drive when we receive it.
